The St. Joseph’s/Candler Center for Medication Management has earned the designation of Anticoagulation Center of Excellence from the Anticoagulation Forum for providing the highest level of care to patients taking antithrombotic medications.

“We are dedicated to improving the health of our patients by collaboratively participating in all aspects of their care,” said Heather Seibert, PharmD, Manager of The Center for Medication Management. “I am very proud that our specially-trained co-workers worked so hard to pass the Anticoagulation Centers of Excellence assessment.”
This is the only Anticoagulation Center of Excellence in the region.

The St. Joseph’s/Candler Center for Medication Management works with patients to better understand their medications and ensure that they remain healthy and do not have to be readmitted to the hospital. The Center has expanded rapidly in recent years to treat many different conditions, but began as an anticoagulation clinic helping patients on blood thinners.

Because it successfully met the rigorous standards in each patient care pillar in the Anticoagulation Forum’s assessment, The Center for Medication Management may be considered an Anticoagulation Center of Excellence for two years.

The Anticoagulation Centers of Excellence program helps healthcare professionals provide the highest level of care and achieve the best possible outcomes for patients on antithrombotic medications. The program offers a roadmap to consistent, sustainable excellence in five key areas of patient care.

“The assessment and educational resources on the site helped us streamline our operational performance, improve transition of care and knowledge of drug therapy, and enhance patient education while learning more about disease state management,” Seibert said. “Until now, there has not been a comprehensive standard to define ideal practices for an anticoagulation service or a central place to find relevant resources for our practice. This educational program will have a lasting and profound effect on anticoagulation care for many years to come and we are proud to be considered an Anticoagulation Center of Excellence.”
